Your First Visit
As a new patient of Dr. Stacy Johnson Family Dentistry, we are pleased that you have chosen us for your dental care and we will make your visits as pleasant and relaxing as possible while providing first class dental care.
You can expect that there will be some forms to fill out including personal information and insurance. For your convenience, we have made the forms available online so you can print them ahead of time and fill them out at your leisure prior to your first appointment.
Your first visit will include a complete oral evaluation (x-rays, photographs, cancer screening and periodontal screening) to establish a baseline, followed by a consultation explaining your diagnosis and treatment options. You might receive treatment the same day and additional appointments may be necessary.

New Patient Forms
New patients are requested to download, print and fill out the forms in our New Patient Packet. The packet includes additional documents that explain Dr. Johnson's Family Dentistry policies and practices.
Financing Options
Obtaining optimal dental care and health is our utmost priority and wish for every patient. In order to make the process affordable and convenient for every patient we accept the following payment options: Cash; Visa, Mastercard, and Discover Card in addition to insurance billing and other payment options.

Online Payments
For your convenience, we offer you the ability to pay your dental bill online via Smile Dash. More than just a means to make a payment, Smile Dash is a free and easy portal to access our front desk services from your computer as well as review your bill.

Insurance
If you have dental insurance, you can expect the carrier (insurance company) to reimburse you for a portion of our fee. As a convenience to you, we will hep process your insurance claims including providing necessary documentation, test results that may be required.
