You are requested to print and fill out the New Patient Welcome packet (see below) at your convenience and bring it to our office for your first visit.

This packet contains our Patient / Insurance / Dental History / Medical History information form, Patient Consent and Disclosure of Protected Health Information, and an informational page pertaining to financial payment options. Please print all pages, fill out the questionaire, and sign all pages. Bring the packet with you to your first visit.
